CIGC’s hysterectomy patient from Maryland – Tracy J. – was struggling with fibroids that caused severe GYN symptoms. Tracy had not been sick before and she first ignored the signs when she first experienced pain and discomfort back in 2012. As years passed, she became extremely anemic due to very heavy bleeding and developed a small embolism in her leg that could have been fatal.

“The day I had to call my family and tell them that I was in the hospital with a blood clot, the hospital staff kept telling them that I could have died, which didn’t help their fears. I had an 8-week course of iron infusions. I had a baseline of 5. It’s supposed to be 12.  I realized after that I had adapted to the amount of iron in my system.” Tracy said.

Paul MacKoul MD Explains How GYN Bleeding From Fibroids Can Cause Anemia

A friend spoke to Tracy about us years ago and she decided to reach out and seek surgical treatment when her symptoms got truly severe.

“At this point, I was having this horrendous bleeding, even though the clotting was managed, the bleeding was out of control and my stomach was always swollen. I now had a new job and great insurance, so I called The Center for Innovative GYN Care.”

“After the surgery, I am a one-woman marketing team, a grassroots campaign manager. I took a stack of business cards and I give them to people who I know have GYN problems. I’m happy to be here. I don’t believe in wasting a day. I’ve lost friends at young ages and have known people who have lived into their hundreds. I want to be one who lives a long life.”

Many of our patients travel from out-of-town and from out-of-country for advanced minimally invasive GYN surgery at CIGC. Women who don’t have minimally invasive treatment options for their GYN condition closer to home can now benefit from CIGC’s groundbreaking techniques through our travel program. Innovative procedures like DualPortGYN and LAAM were developed by Paul MacKoul and Natalya Danilyants to improve surgical outcomes for patients and to help them recover faster.

GYN surgery for complex conditions like large fibroids and extensive endometriosis is now possible with only 2 small incisions and recovery in less than 2 weeks. Many times women are recommended open or robotic surgery and this is not longer necessary with advanced laparoscopic techniques.

With up to 80% of women struggling with fibroid symptoms, many come to CIGC looking for a better fibroid removal surgery. When it comes to surgical treatment, many procedures are still performed unnecessarily as open, and untrained specialists recommend hysterectomy when it is unnecessary. It’s important for women to do their research and get to know their options so they can choose a safer, less invasive surgery.

Women who’ve had LAAM myomectomy with Dr. Paul J. MacKoul, MD and Dr. Natalya E. Danilyants, MD at CIGC talk about their surgical experience and how they have been feeling since.

“No large incision! I have to look for it. I was back to work in 2 weeks. I had 17 fibroids removed. Three of them were really big. I felt excavated, all of the junk removed. It feels so liberating to have them all out!” said Azundai about her procedure with Dr. Paul J. MacKoul. You can read her full story here.

“I had heard and seen what other women had gone through for fibroid removal. Their recoveries were much longer. I really appreciated the small incisions. I compared what I experienced to other women when I got home, and they were amazed.” said Jenny, who traveled from Wisconsin to Maryland for her LAAM procedure with Natalya Danilyants MD. Many CIGC patients struggling to conceive have found that GYN surgery for conditions like fibroids or endometriosis can often clear the path to motherhood. Fibroids affect up to 80% of women and they can interfere with a woman’s ability to conceive and carry to term. Endometriosis affects more than 5 million women in the U.S. and it is the leading cause of female infertility. Fertility investments can be expensive and it is important to diagnose and treat GYN conditions early in order to prevent future damage.

CIGC patients have agreed to share their stories and talk about their journey to motherhood.  Ljubica got pregnant naturally after LAAM myomectomy; and Janelle got pregnant with IVF after minimally invasive endometriosis excision. A common approach to managing fibroids is the “watch and wait method. This is in fact an outdated philosophy as letting fibroids grow can increases the risks of miscarriages and infertility as well as severe GYN symptoms like abnormal bleedingpelvic pain, bloating and uterine distortion. Large fibroids can even put women at risk for a hysterectomy.

The location of the fibroid, and not its size, is the key factor when it comes to fertility. Fibroids growing within the uterine lining, or developing within the muscle, can distort the uterine cavity. The result is often difficulty conceiving and a higher risk of miscarriage.

Current advancements in minimally invasive GYN surgery make it possible to safely remove fibroids with only 2 small incisions and repair the uterus by hand to ensure it heals properly for future fertility. The innovative LAAM technique was developed at CIGC by surgical specialists Paul MacKoul MD and Natalya Danilyants MD.

Gwen-t

Gwen T, fibroids patient at The Center for Innovative GYN Care, had minimally invasive fibroid surgery 4 years ago and feels great speaking about it now. She says she can’t even see where her incisions were and you would never know she had fibroid surgery. Gwen was happy to share her experience with us and talk about how she’s been feeling since.

Back in 2011, Gwen had a fibroid the size of an orange, causing her pelvic pain that she had struggled with for years.

“Every month I would suffer with pain around my menstrual cycle, but it wasn’t in my pelvis. I was getting pain in my hip and my back. There were times I couldn’t get out of bed. At the time I didn’t know what it was, but it was affecting my lifestyle, crippling me.”

Dr. Paul MacKoul performed laparoscopic hysterectomy using DualPortGYN, an innovative surgical technique that uses only two 5mm cosmetically placed incisions. The scars are practically invisible after healing.

“I have no scars. Some women are scared about big scars, but patients considering surgery need to know that I had no drainage, and went home with 2 small bandages. I was back to work in four days. I work as an office manager in a doctor’s office, and it’s light duty, and low stress, so I had no problem returning to work earlier than planned. I’m looking forward to wearing a bikini some day!”

Many women find they have the drive to pursue new activities after recovering from GYN surgery. Following her DualPortGYN hysterectomy, Gwen found the energy to further her education.

“Since my surgery, I was able to get my Masters of Arts Degree and am a licensed Clinical Christian Counselor, with 4 board certifications. I’m pursuing a Ph.D in 2016.”

After minimally invasive GYN surgery at CIGC, many of our patients engage in new activities, travel to places they have always wanted to visit and take more time to spend with their families and loved ones.

Women dealing with fibroids often choose to delay surgery due to fear of open procedures or risks associated with robotic surgery and power morcellators. It is essential that patients do their own research to identify all possible options for gynecologic care. Learning the facts about fibroid surgery and available procedures can help avoid unnecessary pain and get you back on your feet sooner.

3 ESSENTIAL FACTORS IN FIBROID SURGERY:

  • The fibroids themselves:The size, number and location of fibroids determine the type of surgery. Fibroids can occur anywhere in the uterus, most develop from the muscle or myometrium, while cervical fibroids are quite rare.
  • The patient’s future plans: The most definitive fibroid removal surgery is hysterectomyas fibroids often return after surgery. If childbearing is still desired and retaining the uterus is important to the patient, a myomectomy is recommended. It is essential for women who are intending to retain their uterus to have the most minimally invasive myomectomy possible, as the probability of these women requiring subsequent fibroid removal surgery is very high.
  • The surgeon’s skill:Minimally invasive fibroid removal is possible even for women dealing with large fibroids or many fibroids. Fibroid surgery does not have to be limited to open or robotic procedures.

Fellowship trained surgeons of CIGC Natalya Danilyants MD and Paul MacKoul MD have developed an innovative fibroid removal procedure, taking the best of open and laparoscopic procedures, without the use of a robot or power morcellator. LAAM myomectomy  uses 1 tiny incision at the belly button for a laparoscope, and a slightly larger incision, about an inch and a half at the bikini line. The surgeons are able to feel and remove all of the fibroids and patients generally recover in 10 days to 2 weeks.
